This fee will be used towards your treatment. If you arrive late for your appointment we may need to reschedule you.

We accept Visa, MasterCard, Discover, Care Credit, Check, and Cash. We do not accept American Express at this time.
We try to make every effort to keep our menu up to date, but please note that prices and services are subject to change at any time. We love having you as a customer, but we do reserve the right to refuse treatment at any time for any reason.

We respect your time and do our very best to honor our appointment schedule. However, due to the unpredictable nature of our speciality, occasional disruptions do occur. Just as we strive to adhere to our appointment schedule, we ask our patients to do the same. If you need to reschedule your appointment kindly give 24 hours notice. In consideration of other patients, we may need to reschedule your appointment if you are late. It is our policy not to give prescriptions for over the counter medications.

Billing and Insurance Information

It is imperative that we have the correct insurance information for you during each of your visits so that we may bill the correct insurance company. We will help you in any way we can to work with your insurance company to achieve maximum benefits; however, it is ultimately your responsibility to know your health insurance policies and the benefits covered under your plan. While our billing staff can submit your visit to your insurance company, only your insurance company can determine which services are covered benefits under your plan.

Account Balances
Any balance on your account is due within 30 days or at the time of your next appointment unless other arrangements have been made. Please ask about our payment plan policy.

Unpaid patient accounts have become and increasing problem in our office. Accordingly we have implemented a new policy relating to the payment of accounts effective January 1, 2015. All patients will be required to make a full payment of the amount owed, prior to your next scheduled appointment. If you are unable to pay your entire balance in full, we are able to divide it into monthly payments. These payments must be made by credit card that will be run by our office each month. A 1.5% monthly interest rate (18% annually) will be applied to all unpaid balances 30 days after the first statement is mailed out. This new policy includes existing payment plans. IF you feel the patient responsibility as shown on your bill is not accurate, please contact your insurance company to verify.

All co-pays are due PIOR to treatment.

COLLECTIONS: We reserve the right to forward your account to a collection agency if it is determined to be uncollectible. If you're balance is sent to collections, we will add 1/3 of your total bill to the amount to cover the cost of the collection process.
